Documents Required For CUHP Admission 2025

Candidates must bring the following original documents along with one set of self-attested photocopies for verification at CUHP:-

  • Admission Form
  • Two Recent Passport-Sized Color Photographs
  • 10th Marksheet and Certificate
  • 12th Marksheet and Certificate
  • Graduation Marksheet and Degree
  • Caste Certificate (SC/OBC-NCL)
  • Income Certificate (Issued by a Revenue Officer of rank Tehsildar or above)
  • Aadhar Card
  • Notary Affidavit on INR 10 stamp paper (format attached)
  • Bank Passbook Copy linked to Aadhar

Candidates who have already filled the application form through the online portal can directly report to the venue given above for the screening test with their filled application form
